custom .csv files for trades

I have 2 years of trading data from TIAA brokerage. I have a .csv file. If I knew what column headers and data units the file needs to have to upload into Tradevision-Trades, then I could edit the .csv for my file. Any chance you could provide me with a .csv template, so I can load data? The manual entry would be too cumbersome.
